Migrate vCenter 4.0 Standard from one physical host to another physical host with vCenter 5.5

Dear all,

Please advise, steps by steps of Migrate vCenter 4.0 Standard from one physical host to another physical host (different IP Addresss) with vCenter 5.5 Standard.

Existing vCenter 4.0 Standard Configuration

OS: Windows Server 2003 Standard Edition

Database : SQL 2005 Express, bundle with vCenter.
